Botox vs Dysport vs Xeomin — What's the Real Difference?

The short answer: all three are FDA-approved botulinum toxin type A products, they all work the same way, and for most patients the choice does not change the outcome. Where they differ — onset, spread, protein content, price per unit — matters at the margins, not in the mirror.

This guide walks through what actually differs and what does not, so a "we only use Xeomin" or "we prefer Dysport" pitch at your consult is a claim you can evaluate on the merits. Built by a physician with over 20 years of experience in facial cosmetic surgery.

What they have in common

All four leading neuromodulators — Botox (onabotulinumtoxinA), Dysport (abobotulinumtoxinA), Xeomin (incobotulinumtoxinA), and Jeuveau (prabotulinumtoxinA) — are botulinum toxin type A products. They block the release of acetylcholine at the neuromuscular junction, which temporarily prevents the target muscle from contracting. The wrinkle that muscle was creating softens.

Daxxify (daxibotulinumtoxinA) is also a botulinum toxin type A, approved by FDA in 2022. Its formulation includes a proprietary peptide that appears to extend duration.

All are administered by injection with a fine needle. All are FDA-approved for cosmetic use in the glabellar area ("elevens" between the brows). Most are also approved for lateral canthal lines (crow's feet) and forehead lines. The average physician fee for a neuromodulator session in 2023 was $435 across all of these products, per ASPS (ASPS 2023 average surgeon fees).

What actually differs

Onset and duration

Multiple clinical summaries put the typical onset and duration at:

Individual results vary with muscle strength, dose, gender, metabolic rate, and injection precision. Dysport's faster onset is real but small — 2–3 days earlier than Botox on average — and rarely changes the practical experience.

Diffusion and spread

The three "legacy" products diffuse slightly differently after injection. Xeomin spreads the least (roughly a dime-sized area of effect per injection point), Botox somewhat more (roughly penny-sized), Dysport the most (roughly quarter-sized). This matters for:

  • Large muscle groups where broader diffusion is helpful (Dysport is often chosen for the forehead for this reason).
  • Small, precise areas near critical structures where narrow diffusion is safer (Xeomin can be preferable near the brow or around the eye).

For a standard glabellar treatment, the difference is essentially invisible in the mirror.

Protein content and antibody resistance

Botox and Dysport are formulated with accessory proteins that surround the neurotoxin. Xeomin is not — it is often marketed as the "naked toxin" formulation. In theory, the accessory proteins can trigger the immune system to develop neutralizing antibodies, which over time make repeated treatments less effective.

In practice, antibody resistance is uncommon in patients who receive standard cosmetic doses. It matters most for:

  • Patients who have already developed diminished response to Botox after years of use, where switching to Xeomin can restore effect.
  • Patients receiving high therapeutic doses (much larger than cosmetic doses) for medical conditions.

For a first-time cosmetic patient, this is not a decisive factor.

Allergen content

Dysport contains lactose and cow's milk protein. Patients with a documented severe cow's milk allergy should avoid it. Xeomin and Botox do not contain this allergen.

Why units are not interchangeable

This is the single most important thing to understand before comparing prices.

  • Botox and Xeomin units are roughly 1:1 equivalent. 20 units of Botox and 20 units of Xeomin do roughly the same amount of work.
  • Dysport units are different. Approximately 2.5 to 3 units of Dysport equal 1 unit of Botox in clinical effect.

This means a clinic advertising "$4 per unit of Dysport" is not automatically cheaper than one advertising "$12 per unit of Botox." To treat the glabella with Dysport, you need roughly 50 to 60 Dysport units instead of 20 Botox units. The Dysport session at $4 a unit costs $200 to $240; the Botox session at $12 a unit costs $240. In a fair comparison the prices are close.

For a fair price comparison, always compute the cost per treatment area, never the cost per unit alone.

Daxxify: the longevity trade

Daxxify's main claim is duration — around 6 months compared to 3–4 months for the others. That is real. What is less discussed is the trade:

  • Higher cost per treatment. Because you are paying for more months of effect, per-treatment cost is meaningfully higher.
  • Slower to fix a bad result. If the injection produces asymmetry, a droop, or a look you dislike, you wait through more of it before the effect wears off. With Botox, you wait 3 months. With Daxxify, potentially 6.

Daxxify is a reasonable pick for a patient with a long track record of predictable Botox results who wants fewer visits. It is a poor first-time choice.

How to compare a real quote

When a clinic recommends one product over another, ask three questions:

  1. Why this product for this muscle? ("Because we get a rebate" is a wrong answer. "Because I want narrower diffusion near your brow" is a correct one.)
  2. What is the price per treatment area for the recommended dose? Not per unit. Per area.
  3. What has been your previous product and response? If you have a history of good results on Botox, switching brands should have a clinical reason, not a marketing one.

Who should skip this comparison

  • You are a first-time patient with no product preference and no allergies. Whichever the clinic is best-trained with is the right pick. Injector skill matters more than brand choice.
  • You already get consistent results with your current product. Switching creates uncertainty, and the difference in the mirror is small. Stay.
  • You are shopping on per-unit price alone. The unit comparison across brands is misleading. Compare by treatment area or by session.

The rule that survives every quote

The molecule is the same. The results in the mirror are essentially the same. What differs is a small set of technical characteristics that a skilled injector will use to match product to muscle. If your clinic cannot articulate why they are recommending one over another beyond "we use Xeomin," that is a marketing preference, not a clinical one. Ask.

Frequently asked questions

Is one of Botox, Dysport, or Xeomin better than the others?

For most patients, no. All three are FDA-approved botulinum toxin type A products with the same mechanism of action. The differences — onset speed, diffusion pattern, protein content — are small and matter mainly for specific muscle groups and specific patient histories, not for the average forehead or glabellar treatment.

How do they differ in onset and duration?
