Sample Report · Patient: “Diane” · 52
$32,000
Diane had been saving for a deep-plane facelift for two years. After her MirrorMD analysis, we recommended she wait 18 months and spend $5,200 on something else first. Here’s why — and what she’ll get out of it.
Diane, 52
Fitzpatrick II
Symmetry & proportion: 76/100
Face shape: Oval
Section 1 of 4
What she was originally quoted
Diane scheduled three surgical consults in two months. Each plastic surgeon offered her a deep-plane facelift in the $28K–$35K range. Before signing, she ran her face through MirrorMD.
Deep-plane facelift — lower face + neck
$28,000
Anesthesia + facility fees
$4,000
Original total
$32,000

What MirrorMD’s analysis actually showed
Before recommending anything, MirrorMD analyzed her face zone-by-zone. Here’s what we found.
Skin Quality
Significant deficit — the primary issue
Fitzpatrick II with significant photoaging: diffuse poikiloderma across cheeks, mottled hyperpigmentation, telangiectasias along the nasal sidewalls, fine lines at rest, mild crepe texture in the perioral region. The skin envelope is the dominant aging signal in her photographs.
Midface
Mild change, secondary
Mild volume loss in the buccal and submalar regions. Malar prominence is preserved. The midface is aging gracefully and would benefit from skin work before any volumization.
Lower Face & Jawline
Early structural change — not yet surgical
Mild jowling bilaterally; the mandibular border remains visible. Early platysmal banding when smiling forcefully but absent at rest. No significant skin laxity in the cervicomental angle.
Eyes & Periorbital
Stable
Mild upper lid hooding bilaterally, age-appropriate. Lower lid skin is taut without herniated fat pads. Tear troughs mild. No periorbital surgical indication at this time.
Lips & Perioral
Treatable with non-injectable approaches
Vermilion show has decreased moderately with age — typical 50s change. Fine perioral lines at rest. No filler indicated yet — these are skin-quality issues that respond to laser before they need volume.

What we said about her quote
We reviewed each treatment in her original quote against the medical literature and her actual face. Here’s the breakdown.
Deep-plane facelift now
Originally quoted at $32,000
Hold off
Deep-plane facelift is an excellent operation. For Diane today, it is premature. Her structural changes (mild jowling, no significant cervicomental laxity) are roughly 8–10 years away from optimal surgical timing. More importantly, performing a facelift on heavily photoaged skin produces a tighter version of the same skin — the surface texture, pigmentation, and crepe don’t improve, and the surgical scars heal less predictably in damaged skin. We strongly recommend a 12–18 month skin-quality preparation period first. Reassess after.
Halo Hybrid Laser — 1–2 sessions (NOT in original quote)
Suggested by MirrorMD
Single highest-impact intervention for Diane’s actual aging signal. Halo addresses pigmentation, fine lines, and texture in one combined treatment. One session for many patients; some need a second 6–12 weeks later. Recovery is 5–7 days of pinkness. This will move her face forward more visibly than any injectable for her presentation.
BBL Forever Young (Sciton) — series of 3, then maintenance
Suggested by MirrorMD
BBL has well-documented long-term benefits on skin gene expression and pigmentation when done as a series and maintained quarterly. Pairs perfectly with Halo. Often performed in the same session.
Medical-grade skincare regimen
Suggested by MirrorMD
Tretinoin + topical antioxidants + diligent broad-spectrum sunscreen. A skincare regimen done in earnest for 12 months delivers visible improvement on its own and is mandatory before and after laser work to protect the investment.
Section 4 of 4
Her revised treatment plan
Three tiers, in clinical priority order. Skincare comes first because it improves how every other treatment performs.
Tier 1 · At-Home Skincare
Skinceuticals C E Ferulic Serum
≈$182 / 6-month supply
Foundational morning antioxidant. Reduces oxidative damage that drives ongoing pigmentation. Apply daily under sunscreen.
Tretinoin 0.05% (prescription)
≈$30–80 / 90-day supply
Cornerstone of the regimen. Drives collagen remodeling, improves texture, fades pigmentation over time. Start 3x/week, increase to nightly. Mandatory for the 6 months leading into Halo and continued after.
EltaMD UV Clear SPF 46
≈$41 / 3-month supply
Non-negotiable. Every Halo and BBL session you pay for is undone by uncontrolled UV exposure. Apply daily, reapply every 2 hours when outdoors.
Skinceuticals Triple Lipid Restore 2:4:2
≈$148 / 3-month supply
Barrier repair at her stage is essential. Use nightly. Pairs with tretinoin to manage retinization and crepe texture.
Tier 2 · Professional (Office) Treatments
Halo Hybrid Laser — 1–2 sessions
≈$1,800–2,400 per session
The single biggest visual move for Diane in the next 12 months. One session may be sufficient; if not, a second can be done 8–12 weeks later. 5–7 days of social downtime. Performed by a qualified, experienced physician.
BBL Forever Young — series of 3, then quarterly maintenance
≈$400–700 per session
Treats pigmentation and telangiectasias. The published Stanford data shows long-term gene-expression benefits when done as a series + maintained. 4 weeks between initial sessions.
Optional in 12 months: 1 syringe HA filler in submalar region
≈$700–900 (if indicated)
Only if midface volume loss has progressed. Reassess at the 12-month mark with photos for comparison. Might not be needed.
Tier 3 · Surgical
Deep-plane facelift — reassess in 18 months
Reassess in 18 months
Postpone. Diane’s structural changes are not yet at optimal surgical timing. After 12–18 months of skin-quality work, three things will be true: (1) her skin will heal scars better, (2) the surgical result will look more natural because the surface and the structure age together, and (3) she may decide she wants to wait even longer once her skin looks dramatically better. We do not recommend surgery now.
Her revised plan
Halo Hybrid Laser — 1 session (likely sufficient)
$2,200
BBL Forever Young — series of 3
$1,800
Skincare regimen (12 months)
≈$1,200
12-month skin-quality preparation
$5,200
vs. $32,000 facelift today. Reassess at the 18-month mark; surgery may still happen, but with dramatically better skin underneath — and a better, longer-lasting result.
What this saved her
Diane was about to spend $32,000 on a beautifully executed surgery performed at the wrong time. After her MirrorMD analysis, she invested $5,200 over 12 months on the skin work her face actually needed first. When she revisits the facelift question in 18 months, the surgery will work better — and she may find she wants to wait even longer.
